Unexpected Problems - Debris Breaks Loose During "Throttle Up"

The day after Columbia's launch, NASA managers find a troubling anamoly as they view launch videos.  They can clearly see debris which has broken loose from something. 

At this point, they are unsure what it is or whether it has harmed the orbiter.
