Unexpected Trouble - Debris Near the Orbiter's Left Wing

As NASA employees studied the launch videos, and the still shots from them, they could clearly see debris forward of the orbiter's left wing.  Had it struck the leading edge? 

Was the problem just a maintenance issue - or - had a flight-safety problem developed?
