This illustration appears in a very-famous Book of Hours known as Les Très Riches Heures du Duc de Berry.  It depicts the city of Rome and Vatican City (with St. Peter's Basilica). 

Neither Vatican City nor St. Peter's existed at the time Marcus Aurelius was Emperor of Rome.
