The author of this book, David Beaty, is a former Royal Air Force and commercial airline pilot.  He assesses human factors, particularly in the aviation industry, which cause people to make mistakes.

Sometimes seemingly inconsequential errors lead to horrific consequences.  Using expert analysis, Beaty examines events behind some of aviation's worst accidents. 

The disaster at Tenerife, and what caused it to happen, is included in this book.
