The Black Death - What Was It?

Life at the beginning of the 14th century, in Europe, was relatively good.  Then ... a terrible disease, variously called "The Black Death," "The Great Pestilence" and "Bubonic Plague" swept across the continent.  Within the span of five years, untold numbers of people were dead.

What was this terrible illness?  Where did it originate?  Whom did it strike?
